We are not a fee-for-service practice; we benefit economically only if we deliver high-quality patient outcomes and satisfaction. As a result, our clinical teams can spend quality-time caring for a smaller number of patients, giving all patients the space, respect, compassion and care they deserve. Our model is finding success throughout the country; we are now the nation's largest risk-based, in-home medical group, with operations in six markets and four states across the country. At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health and other employed team members. The Community Health Advocate will be responsible for enrolling patients into the Landmark Health program in a face-to-face environment. This role is required to engage patients that have either requested to be contacted in a face-to-face setting, or have proven to be unreachable through telephonic and/or mail communications. Responsibilities include developing local strategy and tactics to reach the member population, including working with the General Manager and Regional Medical Director on PCP and provider outreach, and developing outreach strategies to community based services. This role is responsible for regular reporting on production and progress. Responsibilities Enroll patients into the Landmark Health program, primarily through face-to-face engagement strategies Identify innovative engagement strategies to enroll patient's into the Landmark Health program Assists with patient engagement for those that are unable or unwilling to be reached by telephonic and/or mail communication Establish strong relationships with Landmark Health's target population to help authenticate programs that may be new to the market Establish strong relationships with local community based services Maintain and ensure the confidentiality of all Protected Health Information Flexibility to work extended hours to accomplish business goals Qualifications 2
years of related experience, with prior involvement in a community advocate or Medicare Advantage sales role preferred Bachelor's Degree in public health, health communication, or related field Demonstrated networking and negotiation skills Demonstrated public speaking and presentation skills Excellent interpersonal skills and proven ability to adapt to varying communication styles Ability to work in an entrepreneurial environment with minimal supervision Proven experience communicating with seniors in a face-to-face environment Ability to multi-task and work in a dynamic environment Conversational understanding of medical terminology We can recommend jobs specifically for you! Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
